South West Caregiver Support Fund

The South West Caregiver Support Fund is an interim one-time assistance of $800, to help caregivers with self-care and caregiving expenses.

 The eligibility criteria and administration of the fund is as follows:

• Resident of South West District;

• Applicant or immediate family member is a Singapore Citizen;

• Per Capita Household Income (PCHI) of $1,500 and below;

• Caregiver and Care Recipient must reside in the same household;

• Care Recipient requires assistance with min. 3 of the Activities of Daily Living (ADL)

  • Washing;

  • Feeding;

  • Dressing;

  • Toileting;

  • Moving and;

  • Transferring

or

• Care Recipient certified to have a permanent disability of any one of the following:

  • Physical

  • Visual

  • Hearing

  • Intellectual

  • Autism

  • Others

• Visit your nearest Community Club/Centre

• Complete the South West Caregiver Support Fund Referral Form with supporting documents
